COVID

We are in the 2nd week of Covid. Carol is still symptomatic and contagious.We will continue to work remotely with you until it is safe to return live.

Records release

Medical records are and will be available indefinitely. Release will require your signed authorization. you've been sent that authorization form in the mail. When you find a new provider we can send it to them easily and in various ways. Again there's no rush - your record will be available for at least 10 years.

Medications refills

Please email or leave a voicemail with an accurate list of medications and their dosing which you are requesting. Please include the pharmacy and we will evaluate and fill the your request as appropriate if needed we will contact you for quick appointment

Recommendations for PCP

As for finding a new provider this has been very difficult for some years which is why Carol and I have held on for so long unfortunately it's still difficult. Best suggestions remain asking friends and family for recommendations

Follow up tests and studies that are pending

Any tests or studies that may be pending 10/ 11/20  - will be followed up on You will be contacted with the results.

 If you have a smartphone or access to a smartphone this is easily done with a text sent by us to you

Be prepared with a list of all medications that you will need renewed and a succinct list of questions/ concerns
